Service Coordinator

Maxim Healthcare Services is currently seeking a Service Coordinator for its local office. The Service Coordinator is responsible for accurately coordinating home care schedules to ensure a high degree of customer service. The Service Coordinator works closely with clinical staff members to guarantee accuracy and timeliness of patient care. In addition, the coordinator is responsible for documenting patient information into the homecare system of record to ensure timely business flow. The coordinator is also responsible for daily scheduling and staffing of client services, gathering and recoding initial intake of patient needs, and managing alterations in patient/family requests for change.
Essential Duties and
Responsibilities:
Captures patient intake referral information and communicating information to appropriate management teams
Processes and documents incoming patient needs, requests and changes for services
Correctly informs patients of services available
Collaborates with clinical team to schedule appropriate personnel in a timely manner without lapses or delays in service in the system of record
Records and reports patient and employee concerns, complaints and changes in condition to appropriate management teams
Collaborates with clinical team to track insurance authorizations and doctor's orders.
Enters all appropriate patient information per company policies and procedures in the system of record
Assists office staff members with routine office duties
Performs other duties as assigned/necessary

Minimum Requirements:
High School Degree (or equivalent) required
One (1) year of experience and/or College Degree preferred
Current Licensed Practical Nurse (LPN) licensure preferred
Must be a self-starter with the ability to meet deadlines and excel in a fast-paced team environment
Must be organized with strong attention to detail
Strong time management skills
Leadership and customer service capabilities required
Public relations ability, interpersonal skills, and professional telephone manner required
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required
Advanced knowledge of computer software including Microsoft Office Suite

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
