The CHW supports providers and Resource Coordinators through an integrated approach to care management and community outreach.
As a priority, activities will promote, maintain, and improve the health of patients and their family.
CHW's provide social support, advocates for individuals, and provides education to help people understand their health condition(s).
Essential Duties and Responsibilities Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
Provide ongoing follow-up, via phone calls, and visits to other settings where patients can be found.
Provide referrals for services to community agencies and internal CHC resources as appropriate.
Knowledgeable about community resources in the Three Lower Counties Work collaboratively with other clinical and non-clinical staff assigned to patients.
Act as a patient advocate and liaison between the patient/family, community service agencies and outside specialty providers.
Record patient care management information in the EMR and other software.
Attend regular staff meetings, trainings, and other meetings, as requested.
Manage assigned caseload of patients.
Maintain HIPPA compliance at all times.
Requirements and Skills Proven work experience or willingness to work with people of different cultures and socioeconomic backgrounds.
Understanding of the healthcare system and how to help patients find necessary resources.
CHW certification or willingness to become a Certified Community Health Worker within two years of employment.
High school graduate or equivalent required.
Understanding of the community served.
Basic computer skills with Excel & Word a must.
Preferred experience with electronic medical records (EMR), Outlook and Teams.
Ability to work independently and be self-directed and flexible.
Ability to prioritize and work at a high-volume level of accuracy.
Strong organizational skills to handle multiple priorities while remaining professional Job Type:
